Following Documents in Original at the time of Admission ► 10th,12th Certificate ► BAMS Certificate & Marklists ► Internship Certificate ► Medical registration certificate ► Transfer Certificate and Conduct Certificate( 2 Nos- Issued by previous institution and Issued by Gazetted Officer) ► AIAPGET Admit Card and Score Card ► Allotment Letter (for All india Quota Candidates ) ► KEAM Data sheet , Allotment Memo issued by the CEE ,KEAM Allotted Students ( if applicable) ► Community/Cast Certificate - Only for reservation category ► Fitness Certificate in the relevant format given in Annexure XVII(b) of the prospectus of Govt. of Kerala ► Migration Certificate - In case of student who passed Qualifiying examination from other than Kerala University of Health Sciences ► Eligibility Certificate - In case of student who passed Qualifiying examination from Out side kerala ► Copy of Adhaar Card ► Students have to pay an amount of Rs. 30480/- being the Tution fee, Admission fee and Other special fees at the college Date of Birth Certificate: (If matric(10th) certificate does not bear the same) Conduct certificate 2 Nos ( Issued by previous institution and Issued by Gazetted Officer)
